The Symptom & Root Cause Journal

A 30-day guided tracker for chronic pain

Name
Start date

How to use this journal

  1. Five minutes, morning and evening. Fill in what you can right after waking and right before bed. Don't wait — memory fades fast.
  2. Rate on the 1–5 scale. 1 is barely there, 5 is hard to ignore. Check the box that matches. Don't overthink it.
  3. Note the food, then the symptom. What you ate often shows up hours or a day later. Write both down and let the pattern emerge.
  4. Don't edit the bad days. The hard days are the data. A blank page tells us nothing — an honest 3 is worth more than a polished blank.
  5. Bring it to your consult. After 30 days, you'll see terrain, not just weather. That's where root-cause work begins.

What shifted this week?

What flared or repeated?

Which symptoms came back? When did they show up? What was happening before them?

One thing to try next week

Based on what you saw, what's one small experiment for the week ahead? (A food to remove, a bedtime to test, a stress practice to add.)

Monthly Review

Days 1–30 — reading your terrain

You've gathered 30 days of data. Now step back and read the terrain. These prompts help you see the patterns that only emerge at the 30-day scale — and they're exactly what we'll read together in your consult.

Your pain terrain

Across 30 days, where did pain live most often? What was the average intensity? Did it move, or stay in one place?

Food correlations

Which foods showed up before your worst days? Which ones preceded calm, low-pain days? List the top 3 suspects and the top 3 allies.

Sleep & recovery

How did sleep quality track against pain and energy? Did better sleep predict better days? What bedtime patterns helped most?

Stress & triggers

What stress patterns preceded flares? Which triggers showed up again and again? What single stress-reduction practice made the biggest difference?

What to bring to your consult

List the 3 patterns that surprised you most and the 2 questions you most want answered. This is what we'll read together.

"Your body keeps a record. Thirty days of honest tracking is how you start reading it."

— Tammie Dowden, Peacefully Stronger Wellness
